Many health plans and provider groups offer in-home visits in order to better understand the health and wellbeing of each member.
The visit is in addition to the care provided by your primary care and specialists. During the visit, you can expect a complete history and limited physical, a review of all medications, and a home safety evaluation. The visit typically takes 45 minutes to an hour to complete and will pair you to a clinician who will come to you.
